Five Decades Later, Medicare Might Cover Dental Care: The proposal, part of the large budget bill moving through Congress, would be among the largest changes to Medicare since its creation in 1965 but would require overcoming resistance from dentists themselves, who are worried that it would pay them too little.

The pube lickin's will probably claim Medicare covering Dental Services is a form of creeping socialism even though the recipients paid into Medicare for decades before getting a dime out of it.

The government underwriting an insurance policy that pays for you to go to a private dentist, that is NOT socialism. Now if the dentist was a government employee, now that would be a different situation altogether.

I love it when someone on the Right is telling all of us how terrible socialism is and that we need to immediately remove all instances of it from American society. And then they turn around in the next breath and praise our first responders, the police and firefighters, and our men/women in uniform and demanding that we spend more money on the VA for doctors and hospitals. I'm not sure that these people would actually want to live in America if all the 'socialist' organizations were to suddenly disappear.

"Oh wait... Looking ahead to medicare for all, it has to be changed. Must be a good idea after all."

No, of course it isn't Phester. It is so much smarter to have private corporations control healthcare and carve 20% of our payments out as profits for themselves. Hey, why not pay into a system that employs thousands of people who are only employed to try and figure out ways to deny you coverage? Brilliant system brought to you by.....who else....Republicans.

I don't know which party gets the credit for the creation of health insurance but Obamacare cemented this system into place...you know...the one where private corporations carve 20% of our payments as profits....

Wait though.....I thought Obamacare put a stop to those shenanigans? In fact, in defense of Obamacare, you have repeatedly said so. The profits were reduced.

What gives? Now you're back to claiming 20% profits?

#7 | Posted by eberly at 2021-08-30 09:01 AM | Reply

"...you know...the one where private corporations carve 20% of our payments as profits...."

Not quite.

They are required to pay out 80% of premiums on claims. This is the Medical Loss Ratio.
But the remaining 20% isn't pure profit.

You know what Cigna's profit margin is? Hint: a lot less than 20%

Businesses have overhead, did you know that?

#8 | Posted by snoofy at 2021-08-30 09:09 AM | Reply

We could jack up the Medical Loss Ratio to ~95%, which would still allow Cigna and all the rest to skim just enough operate -- and our health care system still wouldn't be cost competitive with a single payer system.

Eberly, please explain why.

"resistance from dentists themselves, who are worried that it would pay them too little ..."

Dentists hear MDs complain constantly the low reimbursement rates from Medicare.

Dentists that are busy and dont' need to fill up their schedules with new customers (such as medicare recipients with no other means to pay) already get more $$ from older folks with no dental insurance.

This would cut into their revenue...or they fear it would.

My wife and I have been going to a dentist an hour north of us for routine cleanings and such for quite a few years. He makes a good living and doesn't try to rip people off. A couple years ago while I was getting a cleaning he said I needed a root canal, my first. 10 minutes late it was done. $50. They charge hundreds for the same procedure here in Nashville.

Don't listen to dentists who claim Medicare won't pay them enough. They and veterinarians now think they should all be living in mini mansions.

"resistance from dentists themselves, who are worried that it would pay them too little ..."

This is a valid concern.

But -- get this -- it's also a concern that had already been thought of in the states that have chosen to make dental coverage a part of Medicaid.

We did some work on this at my last job. Older adults mostly just need extra time in the chair, which you can adjust for by applying a modifier code to the billing paperwork.

The care model they have in Minnesota is probably sufficient to blueprint a Medicare dental benefit.

We need to get past the "healthcare for profit" notion here in the United States.

A Country should want its citizens to be healthy, and should not need to rely on the profit-thirst of greedy corporations to do attain that goal.

Pretty sure it is 90/10 already. The problem is there is an unintended consequence and it got to some of the companies and the payments they negotiate with hospitals and such that are high. That is that there really is no incentive to control costs because the higher the cost the more they can charge everyone and therefore the greater the size of their 10% cut which directly goes to profitability.

For Eberly, the system was pretty concrete for decades. The ACA just made it so everyone had to get covered and put limits on much of what corporations were doing. Like famously per-existing conditions coverage.

Moving to a single payer system still seems the best bet. There of course would be the potential for the bloodsucker insurance companies to come in with additional coverage as a possibility with private care as well.

#16 | Posted by GalaxiePete at 2021-08-30 12:22 PM | Reply

Frankly it is about time on dental coverage. I can't believe what they charge for an extraction, crown, implant, etc today. If you have a tooth go bad you are looking at thousands without an implant. Seriously - Over $1000 for a simple extraction or a root canal. Crowns - $1000 and up. I don't know what a bridge costs anymore but has to be pricey. A single implant from someone very inexpensive is $1500 or MUCH more.

The payer-provider model is supposed to address this problem. Since you can't rob Peter to pay Paul, when they're the same person. It's as close to a single payer model as we can get.

But, a handful of payer-provider models in a sea of decoupled payer and provider entities won't move the market.

One of the often overlooked advantages of treating Medicare patients is that the doctor KNOWS up front that he's going to get paid. No deadbeats, no turning people over to collection agencies and accepting pennies of the dollar to clear their accounts receivables, no having their patients hounded by debt collectors resulting in them losing a patient, etc.

None of the doctors that my wife and I have gone to in the last 5+ years, that we've been on Medicare, appear to be barely hanging on financially. They all have nice, modern well appointed offices, they drive expensive cars, their kids attend world class universities, etc.

For a moment, set aside medicare coverage for dental services.

For all the years I've purchased and in years past sold group dental coverage and studied the forms and coverages typically available in those policies and comparing it to the premiums.....let me say this....for the premiums charged, I wouldn't buy dental insurance for myself and my family if I had to pay the entire premium.

The annual cost for a family compared to how it's utilized......it's not a good insurance buy. Now, if you have a lot of teeth problems in your household and a group plan that can't single you out for your individual dental issues...then maybe it's a better buy.

But for a family with relatively health teeth who rarely needs dental coverage for anything beyond the semi-annual check-ups/cleaning/x-rays? For what I could negotiate with a dentist as a cash buyer....I would self-insure. Even if we had some occasional cavity, crown, etc work done....it's still better to pay cash and keep your premiums.

#29 | Posted by phesterOBoyle at 2021-08-30 02:03 PM | Reply

#29 teeth aren't necessary, we have dentures.
At least, that's what Medicaid will tell you.

Much cheaper to extract a tooth than give it a crown, and you can forget about a root canal.

So they just pull them, even when they could be saved, and fit a partial denture.

Basically, if it can't be fixed with a filling, they pull it.

We ought to be able to do better than that.

#30 | Posted by snoofy at 2021-08-30 02:38 PM | Reply

The root canal the great dentist an hour of Nashville only charged me $50 (and took 10 minutes) for had only contained one difference from having a cavity filled:

- After drilling, he put a threaded device into the hole to pull the root out. Took a minute, tops.

Other than that, there is no difference between having a cavity taken care of and a root canal. Same drilling, same filling.

Here in Nashville? It's hundreds up to a thousand dollars for a root canal.

More like a root canal of people's wallets.

Root canal and cavity drilling are not the same at all. A root canal is drilled all the way to the bottom of the root of the tooth. Cavities are much shallower.

It took a minute longer than having a cavity drilled and filled. And that was only for the hand-turned threaded device he used after drilling and before filling.

His threaded device had a handle on top - like a fence post digger. No delicacy necessary.

As I said, he was done with that bit under a minute. Everything else was like having a cavity drilled and filled.

So, why do dentists regularly charge hundreds to over a thousand for a root canal? It wasn't until I had a root canal done on myself I knew dentists are ripping people off.

The reason an insurance company (not Medicare) doesn't cover hearing aids is because they can't actuarial it out to be profitable.

It's not that kind of risk. And since it's not life threatening like other health events it's considered elective. A lot of health issues could be considered "elective" as well but those risks are easier to price.

Personally I think hearing is a health issue and an important quality of life aspect. I know people who need hearing aids but are too cheap to get them.

My hearing aids have made my life safer as well. I have one really bad ear and one bad ear. So it was impossible to tell the direction sound is coming from. However, another wonderful feature of these utilizes the two microphones. With a total of 4 mics, when a sound is coming from off center (not forward), the computer knows this and amplifies the side the sound is coming from more than the other side. The hearing aids have to talk to each other of course, which they do. This gives me a more natural sense of stereoscopic hearing and the ability to recognize the direction of sound.

The technology is amazing.

Do you know how much heart disease comes from poor diets? Should the government provide healthy meals to all? Even more heart disease comes from smoking and the government actually subsidizes it! Do you really think they care about heart disease?

I think tobacco subsidies should end before paying for dental. And if we must live in a nanny state, healthy meals -- which provide a plethora of health benefits -- should be made available to everyone for free.

I see zero downside towards feeding the needy.

And, since literally everyone needs healthy food, healthy meals available to all is an idea I can get behind.

Doctors who deal with nutritionally deprived people would absolutely Rx meals if they could.

Children from birth to age 5 will suffer 10-20 points of IQ loss and exhibit more violent and aggressive behavior if they are malnourished. Proper nutrition is probably one of the most important health interventions a society can make.

Worst of all, this ridiculous persona is bereft of the stuff people liked about you -- only the troll remains.

Beyond that, I doubt I even trolled you. But since it's such a pivotal moment in your life, tell us what you wrote and what I wrote back.

Eberly's first post in this thread was (checks notes) to proclaim his ignorance about the history of health care, and then take a dump on Danni because Democrats passed Obamacare.

Eberly, you should be aware that FDR wanted a national health program like they have in the UK. To fight this, the AMA invented the term Socialized Medicine to demonize it. Later, when LBJ passed Medicare, they tried to demonize that too. And the same thing happened when Obama tried to reform health care. Somewhere in the middle there, Hillary Clinton tried some reform as First Lady, and that too was demonized. And always by Republicans. Democrats try reform, Republicans cry Socialism. Over and over and over.

You really didn't know all this? It seems hard to imagine you don't, considering how much the topic comes up. It's been talked to death here.

Well, now you now. Stop playing dumb.

There is no million dollar baby in dentistry.

About the most expensive thing a dentist could do is replace every tooth with an implant.

You take your liberalism very seriously. So seriously that you take great offense to such a comment.

-Have you found any lefties who check all the boxes?

Has anybody here ever followed you from thread to thread to thread every time you ever post here?

follows up everything you post with at least 2 retorts? Then goes on and on and on after you leave? Can't engage in any dialogue, then keep asking why you have a "D" after your name? over and over and over and over? Blaming you for the power 1 party has. It's your fault?

That's my list, Danforth. And I've got MULTIPLE people who can check all those boxes.

Dentistry prices have come down remarkably since I've been a customer, in todays dollars. And quality and technology has gone way up. Especially with the 3d surround visual whatever that is. Now I can see the $1800 crown and root canal job I got 30 years ago was actually a ------ job. Plus there seems to be multitudes of dentists these days.

The truth is that we only care about our own pain here.

We don't see others dealing with it. So maybe Goatman was just as bad as Snoofy.....but not worse.

And here is the consequence of it.....the more snoofy posts...the less a lot of other much more worthy posters do.

Corky
Doc
Danforth
Twin
Gal

There are more...but that's just off the top of my head. I think all those posters are here less in part because what snoofy has done to this place.

The self-exiled are gone in part due to snoofy. Not entirely but it's not insignificant either.

An observation:

Folks here don't like to be put on their killfile list. I posted consistently since I came here. But after I grew weary of certain posters for their trolling, hostility, and lack of substance, suddenly there was this "Jakester is a sock puppet", "Jakester is a big ----- for plonking", "Jakester is a fake", blah, blah, blah. I never changed my posting ideologies. But as soon as I announced I was plonking people, the hostility began.

So the logical conclusion is that people don't like plonking. They seem to think I (and probably others) are obliged to put up with --------.

B ------- S.

Dentistry is remarkably cheap. Insanely cheap.

There is no million dollar baby in dentistry.

About the most expensive thing a dentist could do is replace every tooth with an implant.

And that costs about as much as a new truck, or a few days in the hospital.

You're completely clueless if you think dentistry is expensive in the context of overall health care expense.

#150 | Posted by snoofy at 2021-08-30 11:45 PM | Reply | Flag:

When 40% of Americans can't afford a $500 car repair bill a new truck is insanely expensive to them.

Back in 1995 my dentist died and his practice was sold to dental practice in the burbs...I pull up and see reserved spots for the "doctors" all with high end Mercedes in the spots. At the end of the check up the "doctor" (his words not mine) comes in and shows me two xrays that look exactly the same... "see this?" pointing to the gum line. No, what should I see? "See how it's not like the other side of your mouth? It means your gums are receding and you need surgery to correct it. I can schedule you for next week. Normally this costs $5,500, but if you book it today I can knock $900 off." I right then and there called him a con artist to his face. The dental assistant in the room looked like he was going to explode and quickly closed the door to the room. I told him that I am not spending $5,500 without getting a second opinion and his "buy it now" pressure was tacky and offensive. Went to another dentist and had him check my teeth...nothing wrong. It is now 25 years later and I have not lost any teeth and never had the surgery.

For people that cannot afford a $500 unexpected bill, $4,600 might as well be $460,000.

Yeah I didn't get taken by that fraud, but not everyone will dare ask for a second opinion when it will cost $200.

I know a dentist that has no shame. It's all about the money and nothing else. He has no pride in his profession. Doesn't care about patients or his staff. It's all a means to an end.

His lifestyle.

"When 40% of Americans can't afford a $500 car repair bill a new truck is insanely expensive to them."

Of course.

Health insurance costs about $500/mo.
Dental insurance costs about $20/mo.

And some of those cheap dental problems lead to much more expensive health problems.

Eventually, somebody pays.

Our health care system chooses to pay a lot later, instead of paying a little now.

A couple of weeks ago I ceased posting here for several days in hopes the obsessed ones would get over their preoccupations with me and move on. When I came back, I posted as always, not making personal attacks, not engaging in name calling, and pretty much sticking to the topic. (I confess, I succumbed to topic drift, but as in attacking another poster) It appears my big sin is disagreeing with the echo chamber and behaving like a liberal and not a toxic mouth-breathing progressive and (through no fault of my own) assumed to be one or all of about 5 or 6 different posters. The triggered were retriggered.

I was thinking of making another sojourn. But as they say, doing the same thing and expecting different results is the hallmark of a fool.

So I have a proposal that could work: Is it permissible to make a topic about a poster so that people who feel compelled to talk about him can go there to do so rather than hijack a thread to talk about him? This would help a lot, I think.

Definitely.
Nobody on the left does this.
Trump pretty much enshrined it as a first line of defense gaslight tactic, with his comments about the Unite The Right rally in Charlottesville.
The one where there were "good people on both sides," even though one side was made up entirely of racists, neo-Confederates, and White Nationalists.
