Nobody is trying to make you think "a man in a dress" is physiologically the same thing as a girl who grew up to be a woman.
That's why we call her a trans woman. It kind of already accomodates your concern about her not being a "real woman."

You seem to get that you can accept them as women and also accept that they were once, physiologically, a little boy.

Not a trans woman among them.
