Let's say you need a drug like Humira that now costs something like 6900 per month. You also have ------ Obamacare insurance that costs 2400 per month for two people and the specialty co-insurance is 50% and the max out of pocket is 18000. Do you really have healthcare? The drug company has a program where they will pay your co-insurance up to about 15000 per year. After that you would think that your max out of pocket and deductible have been met. But no, the pharmacy benefit manager (owned by the insurance company) has a copay accumulator in place. That means when the drug company's subsidy runs out, you still haven't met your deductible and max out of pocket. Never mind that in order to get placed on the PBM's formulary the drug company has to pay rebates to the PBM. Your co-pay is based on the list price, not the price less the rebate so the PBM double and triple dips. Some drugs are less than a penny a pill but you still have to pay the copay based on an imaginary list price that no one actually pays.
