#30: There are three small arteries that feed blood to the outer ear. See image in link below.

If a 5.56 round struck an object near him, the tiny debris from the chair, teleprompter, or dais could have struck one of those arteries like a hot pin. Anyone near that bullet impact would receive similar splinters or fragments. Whatever type of wound #45 received would be towards the top since the blood appears to be flowing down and around. When women have their earlobes pierced, any bleeding that might occur is at the bottom.

A bullet graze would be damaging because of the force and heat of the round, so I would imagine some loss of soft cartilage flesh from the ear, unless the distance of the trajectory was that far, thereby it's a miss. But would someone react to a miss by touching his ear in curiosity? He might have been grazed with the fired round one millimeter away from his flesh, but I am doubtful.

There are medical people on DR who have treated gunshot wounds and could analyze this better.

Also note #45's face appears stunned or shocked. Look at his eyes.

Lastly, the three horizontal red streaks on his face are curious which is what I'm pondering now.
