It's possible that the Russian missile was targeted elsewhere and was damaged by a SAM and hit the hospital instead.

During the Iraq war (the first one) Scuds were frequently hit by Patriot missiles but the warheads still hit something anyway.

Agreed. Russia is claiming it was a NASAMS fired by Ukraine. NASAMS isn't even a SAM, it's a ground based launcher for air to air missiles like Sidewinders and AMRAAMs which have a 50ish pound warhead.
