The Chief Controller of the Facility, the Training Officer, and every Team supervisor must be held accountable.
Local procedures and letters of agreement must be reviewed for efficacy, and challenged where they consistently impose overly demanding procedures which are failing to benefit the day to day facility operation.
There is way too much tragic failure in that facility, with more yet to be visited upon the flying Public.
Back when I was a controller...well, I know of no major ATC facility which has failed this hard and this often.
ATC priorities, in order of importance: First priority is safe.
Second priority is to provide an orderly Flow.
Third is to provide an expeditious flow.
When a facility is consistently failing the 1st priority, they must be re-examined under a microscope until the solution is devised and implemented.
Failing the Safety priority can not ever be accepted. Facility Management is failing the Controllers and the public at large.
